Welcome to the Skyward 2020 Health Screening Registration

Please note that only employees participating in Skyward’s insurance wellness program may register for an appointment.

Your screening will include: blood pressure, height/weight, and a blood draw. The blood panel will include: total cholesterol, HDL, LDL, triglycerides, and glucose.

Being a fasting blood draw, avoid all food and beverages 10-12 hours before your appointment. Only water is acceptable, and you are encouraged to drink plenty as it will help your blood draw go smoothly. Consult your physician if you have questions about fasting and your medications. It’s best to avoid all alcohol and caffeine 48 hours prior to the screening. Please wear lightweight clothing with sleeves that can be easily rolled up, a short-sleeved top works best.

Results may be picked up at the Skyward Clinic once they’re available, and any results remaining in the clinic after two weeks will be mailed to the address included on the consent form.
